Please note: Intake paperwork and consent forms must be completed at least 48 hours before your appointment, or it may be subject to cancellation. Payment (copay or full fee) is due in full before the session begins.
Follow-Up Appointments (Established Patients)
• Telehealth Appointment – 30 minutes – $375
• Telehealth Appointment – 60 minutes – $750
Nutrition Psychiatry Evaluation (For Established Patients – Cash Pay Only)
• Telehealth Appointment - 60 minutes – $450
